Advance Care Planning Education Workshop (Zoom/English)

It is important for caregivers to help their loved ones figure out the latter's preference for future health and personal care should they ever be unable to voice their concerns.
In this workshop, Denise Perron and Fiona Siebert from Vancouver Coastal Health will go through why and how to do Advance Care Planning together with you.
Guest speakers:
Denise Perron, Registered Nurse working as "Chronic Disease Management Nurse" in the past 10 years. She works with Family Practitioners (GPs) in the community to educate and motivate patients with chronic disease to make healthy lifestyle changes. Previously, Denise also worked as a Critical Care Nurse in Emergency for 10 years, and a travel nurse to many places around the world.
Fiona Siebert, registered nurse of 25 years from VCH, currently working at Chronic Disease Management with the Primary Care Network. Her background includes hospice palliative care, home care and case management.
